The Medication Administration Permission Form is required for any student who will take medication at school. This may include regular prescriptions or over-the-counter medication to be administered as necessary. ALL medications must be kept in the nurse's station and accompanied by the form below. If you have any questions, please contact the school and ask to speak with someone in the clinic.
